You must disclose who your primary insurance carrier is prior to treatment.

All insurance is subject to verification and eligibility prior to rendering services.

The deductible responsibility of the patient is assigned by their respective insurance carrier NOT from us.  

  Deductibles and copays may vary depending on the requirements of your carrier.
 
Please be sure to notify the receptionist if you know you have a secondary insurance policy to avoid delays in processing to your carrier.

Some insurance carriers require pre-authorization prior to treatment in our office.  Please check with your carrier prior to treatment to avoid unnecessary liability.  

If you fail to notify us in the event your insurance changes during the course of your care, you may be liable for the cash rate associated with your visits. 

MEDICARE:  The base deductible for 2025 is $257.  If you fulfill your deductible with us, you are good for the year with all other providers.

Be advised that not all deductibles are covered by your secondary carrier. 

We do our best to verify your responsibility with your respective carrier, however, the explanation of benefits you receive after your claim has been submitted will dictate your financial responsibility. 

You should receive copies of your explanation of benefits after your carrier has processed your claim.  If you have any additional questions or concerns, please contact us.
