All insurance is subject to verification and eligibility prior to rendering services.  Deductibles and copays may vary depending on the requirements of your carrier.

Some insurance carriers require pre-authorization prior to treatment in our office.  Please check with you carrier prior to treatment to avoid unnecessary liability.  

If you fail to notify us in the event your insurance changes during the course of your care, you may be liable for the cash rate associated with your visits. 

Be advised that not all deductibles are covered by your secondary carrier. 

Please be advised that although we do our best to verify your responsibility with your respective carrier, the explanation of benefits you receive after your claim has been submitted will dictate your financial responsibility. 

HPN/BCBS patients are required to pay for their x-rays on the first visit.  If after billing on your behalf, your carrier does pay for the x-rays, you will be reimbursed accordingly.  

Humana patients:  If your insurance card has anything showing HMO, Intermountain, or Optum they are not accepted.
